This patient presented within 3 hours of symptom onset with only minimal changes on non-contrast CT. A very large region of severely reduced CBF (Malignant profile) was identified on the CT perfusion scan (performed at the same time as the baseline CT). Following intravenous tPA treatment the patient developed a very large infarction and brain herniation.
